After the assessment, we create a personalized care plan with recommendations, resources, and next-step guidance. The care plan may include support needs, safety concerns, housing options, care services, and family prioritie

Arrangement of Services

Senior Connect helps coordinate services based on the care plan. This may include home care, placement support, medical appointments, hospital visits, crisis support, family communication, housing transitions, and advocacy.

 

Advocacy and Family Support

We serve as a trusted advocate for your loved one and a guide for the family. We help ask the right questions, communicate with providers, and support informed decision-making during difficult moments.

 

Monitoring client needs

Care needs can change over time. Senior Connect provides ongoing check-ins, updates, and reassessments to help ensure your loved one remains safe, supported, and connected to the right services.​

For older adults who want to remain independent, we can help coordinate additional support such as technology assistance, household services, beauty appointments, dining,  errands, travel arrangements and other quality of life needs.
